Dementia Advisor – 12 months Fixed Term Contract Subject to Funding

Responsible to Carers Resilience Manager

 

Hours 19 Hrs (Monday to Friday, occasional weekend may be required)

15 hours CRS contract, 4 hours SPS contract

 

Salary £15,057.12

 

Purpose

With a primary focus on Carers of people with dementia and people living with a dementia diagnosis. To work with carers, clients and PCN’s throughout Rotherham, raising awareness of carer’s issues and providing, or where appropriate signposting to sources of support, information and advice.

 

Working collaboratively with statutory and voluntary partners, to build carers resilience, prevent carers falling into crisis and provide practical timely support to Carers who may already be in crisis.

 

Main Duties and Responsibilities

 

· Support carers of people with dementia, including signposting and referring to local and national services

· Promote local services available to people living with a dementia diagnosis to maintain their independence.

· Provide person-centred, tailored support to people living with dementia

· Provide person-centred, tailored support to carers of people with dementia

· Provide home visits to carers in crisis to identify individual need and devise outcome-based support plans.

· Support carers to develop an emergency plan to provide peace of mind and a clear protocol in times of crisis.

· Promote dignity, respect, and choice in all aspects of care.

· To report accident / incidents / safeguarding as appropriate to the relevant agencies.

· To ensure the service delivered is personalised to the carer / client need

· To actively manage a case load, attend case review meetings and PCN meetings and feedback as required.

· To deliver Carers Awareness Dementia Awareness and Herbert Protocol and ‘This is me’ workshops.

· To deliver Carers awareness events for GP’s, staff and health professionals.

· To provide case studies and accurate case notes

· Actively utilise outcome tools, eg Outcome Star to monitor effectiveness of the service

· In collaboration with voluntary and statutory agencies, support carers to access services, training, advice, information and benefits to promote carers health and wellbeing.

· Attend and participate at meetings and forums relevant to dementia and carers issues as required highlighting services at Crossroads care Rotherham.

· To direct all referrals through the Carers Resilience service in line with the referral pathway.

 

 

Personal Training and Development

 

· To keep an up-to-date knowledge base through regular training.

· To establish and maintain effective communication links to other agencies

· To participate in regular supervisions and performance reviews.

· To participate in regular team meetings.

 

Promoting Crossroads Care

 

· To represent and positively promote Crossroads Care Rotherham locally and the services available to carers living in Rotherham.

 

 

Other Tasks

· To follow Crossroads Care Code of Conduct.

· To work within the philosophy and policies of the organisation.

· To work within Health and Safety regulations.

· To ensure that equality and diversity principals are applied at all times.

· To ensure confidentiality is maintained at all times.

· To undertake any other duties commensurate with the post holder’s grade as requested by the CRS Manager.
